Your First Visit

Welcome to Oral & Facial Surgery Institute! Thank you for choosing our practice for your oral surgery needs. When you visit us for the first time, you’ll have the chance to meet our team and your surgeon, and share more about why you are seeking treatment.

We’re excited to get to know you. Before you come to our practice, we ask that you take a few minutes to fill out patient registration forms. If you are unable to complete these forms, please arrive early to complete the paperwork.

What to Bring

Please bring the following items to your first appointment:

• Referral slip and X-rays, if applicable
• A list of current medications
• Medical/dental insurance information
• Valid identification (e.g., driver's license)
• Form of payment

Please Note: A parent or legal guardian must accompany any patients under the age of 18.

What to Expect

First impressions are everything. That’s why at Oral & Facial Surgery Institute we want to make your first appointment as seamless as possible. At this appointment, you’ll be able to meet Dr. Davis, our seasoned oral surgeon who will work with you to craft a treatment plan designed to meet your unique needs. He will perform an oral examination, review relevant health history, and learn more about your goals to recommend the most appropriate treatment.

Although some treatments may be performed during your first appointment, most require a second appointment. If this is the case, we will schedule your appointment and provide instructions and other information to prepare you for your surgery.

3D Imaging

By using the latest CBCT technology, digital X-rays and impressions, intraoral scanners, and guided implant placement, we’re able to provide a highly accurate treatment plan based on the information we receive from crystal clear imaging.
