We’re excited to get to know you. Before you come to our practice, we ask that you take a few minutes to fill out patient registration forms. If you are unable to complete these forms, please arrive early to complete the paperwork.

A parent or legal guardian must accompany any patients under the age of 18, and during the consultation you’ll meet the team and surgeon to discuss your treatment needs.

What to Expect

First impressions are everything. That’s why at Shawn B. Davis, DMD, Oral & Facial Surgery Institute, Dental Implant Specialists, we want to make your first appointment as seamless as possible and help you understand when to visit an oral surgeon for specialized surgical care. At this consultation in our office, you’ll meet Dr. Davis, Southern Utah’s best oral surgeon,

and learn what to bring, including a valid photo ID, dental insurance cards, and a complete list of current medications. He provides specialized care as an oral surgeon, and will work with you to craft a treatment plan designed to meet your unique needs, which is different from the routine care you receive from a general dentist; understanding the difference between an oral surgeon and a dentist can clarify why you were referred here. He will perform an oral examination, review relevant health history, discuss sedation options, and learn more about your goals to recommend the most appropriate treatment from our full range of oral and maxillofacial surgery procedures.

Although some treatments may be performed during your first appointment, most require a second appointment. If this is the case, we will schedule your procedure and provide instructions and other information to prepare you for your surgery

By using the latest CBCT technology, digital X-rays and impressions, intraoral scanners, and guided implant placement, we’re able to provide a highly accurate treatment plan based on the information we receive from crystal clear imaging.
